What should I bring to my first appointment?
Arrive 15 minutes before your appointment. Some simple forms will be
required to be filled. Bring your current list of your medications.
How will I pay for my care?
If you have health insurance, please present your membership card when you
check in. Depending on the terms of your agreement, we will collect your
co-pay or bill you through your insurance plan. If you do not have health
insurance, we require payment at the time of service. We strive to have
all aspects of our practice, including our professional fees, reflect our
ethical and rational approach to your care. We also accept a cash base for
individuals without insurance. We also accept well paid subscriptions for
certain services.
Do I need a referral to make an appointment?
We are currently accepting new patients and you do not need a referral.
Do I need to have my blood pressure checked regularly?
There are several factors that warrant regular blood pressure check which
can be influenced by age, family history, risk factors such as smoking,
obesity and more.
How can I get a hold of my PCP?
We take great pride in our Patient-Provider relation relationships and
therefore schedule plenty of time to answer all of your questions during
your appointment. Nevertheless, If you need to get in touch with your
primary care provider in between appointments, please contact our main
office to leave a message or schedule an appointment.
How do you notify your patients of lab or test results?
We are usually able to notify patients shortly after receiving results by
telephone or the patients preferred way of communication.
Will I be able to get copies of my test results?
Yes, upon request we are able to provide you copies with your test results.
What is your cancellation policy?
We require a 24 hour notice to cancel all appointments. Please be sure to
contact us if you will not be able to make your scheduled appointment to
avoid cancellation charges.
Do you allow urgent or same day appointments?
We do set aside time for urgent visits. However, if you are having an
emergency, please dial 9-1-1.
